VACCINATION UPDATE

Per the Florida Department of Health (DOH), residents and visitors at least 65 years old may now contact the Florida Department of Health Office in Levy County to make a reservation to receive the COVID-19 Moderna vaccine.
To make a reservation, call 352-486-5300. Due to heavy call volume, phone lines may be busy. If there is no answer, please wait a few minutes and try again.
Please DO NOT contact Levy County Emergency Management. We will not be able to assist in getting you an appointment. The vaccination process is handled entirely by the Florida Department of Health.
You can however continue to monitor our website at www.levydisaster.com where we will post all updates as they are received.
SCREENING AND CONSENT FORM
FDOH has provided me with the screening and consent form that is okay to be published. Those that make a reservation for a vaccination can view/fill it out before arriving on their given reservation in order to save time.
If someone does decide to fill it out ahead of time, please note, FDOH does not need them to fill out the insurance section. They are not charging for the vaccine.
