In this time of worldwide health crises, Three Rivers Legal Services is available to make sure that individuals in our community are prepared with end‐of‐life documents. This one small step can allow individuals to make their own decisions about their medical treatment.

Three Rivers Legal Services has staff and volunteer attorneys available to help answer questions about Advance Directives, the documents needed to let the medical professionals know a person’s health care wishes. At this time, the two most important documents are the Designation of Health Care Surrogate and the Living Will Declaration. These legal documents direct family, medical professionals and friends at a time when the individual may not be able to speak for themselves.

A Health Care Surrogate is an adult who is appointed to make healthcare decisions for the individual who cannot make those decisions for themselves. A Living Will states the individual’s choices for end‐oflife medical care.

Three Rivers Legal Services is a local non‐profit law firm and provider of free civil legal aid in 17 counties in North Florida.
