Medicare Part D Enrollment Deadline May 15th SHINE (Serving Health Insurance Needs of Elders), a volunteer program with the Florida Department of Elder Affairs, will be available at the sites below to provide information and help to enroll in the Medicare Part D: Prescription Drug Plan. Wednesday, May 10 10:00 a.m.-Noon. Chiefland Senior Center 2:00-4:00 p.m. Cedar Key Public Library Thursday, May 11 10 a.m.- 2 p.m. Williston AARP Health Fair First United Methodist Church, Williston Bring your Medicare Card and either a list of your prescriptions with dosages and quantities or the bottles. You may be eligible for Extra Help in paying for the plan premium, deductible and drugs. To qualify you must meet the following: Single Income is $14,700 or less Assets total $11,500 or less Couple Income is $19,800 or less Assets total $23,000 or less Assets do not count your home or your vehicles. Ask us for details and an application! After May 15th, there will be a penalty of 1% per month on the plan premium when you do choose to enroll. The next time you will be able to enroll or change plans is November 15-December 31, 2006, the annual enrollment period.
